Requirements for Service

The patient has been given a diagnosis of terminal illness (less than 6 months to live) from a physician

The patient is seeking comfort-oriented care, rather than treatment aimed at cure

The patient and family are informed about hospice and other options and consent in writing to hospice care; the patient's physician also consents to hospice

Hospice must be provided in a safe setting for care

Hospice aims to provide solutions to all problems related to the terminal illness, whether they are physical, social, psychological, emotional, practical, or spiritual in nature, via a team approach

The level of care includes:

Routine care, respite care, continuous care and inpatient care
